You will also need to bring the following items along with your registration forms, either:

• photographic identification, such as a passport or driving license; and
• proof of address (this can be a letter that has your name and current address on and is dated within the last three months, such as a utility bill, bank statement, council tax letter or any other letter addressed to you).
• In addition, if you are not a UK or EU citizen, we will need to see any relevant documentation from the Home Office as proof of your entitlement to access NHS services.
